We hope to make our visit more personal and efficient by having you complete paperwork ahead of time.  Please take the time to review the links below.  Please also bring any immunization records for yourself or your child as well all medications, supplements, herbs or over the counter medications that you take on a regular basis (or a complete list).  We will need a copy of your driver’s license or government id as well as insurance card, if we will be billing insurance, to make sure we are accessing the correct chart on each visit.  Thank you for your understanding.
